
School Based Health Centers
Urban Health Plan’s school-based health center, located at I.S 158 Bronx, serves students from The Dr. Richard Izquierdo Health and Science Charter School, named in honor of our founder, Bronx Latin, and Bronx Career and College Prep High School. The center provides essential healthcare for middle school students including adolescent health care, immunizations, and sexually transmitted infection (STI) prevention, in addition to:
Psychological and Social Work Services: Emotional and social support to address challenges.
Health Education: Teaching students to make informed decisions about their health.
Dental Care: Providing easy access to much-needed oral health services.
Our bilingual, experienced team understands the unique needs of pre-teens and teens. The center offers walk-in care for all students, regardless of their ability to pay, making it an excellent resource for families.
Children are important members of our community, and their health is our priority!
Partnership with Urban Health Plan
UHP is a not-for-profit system of twelve health centers, one mental health center, twelve school-based health centers and three part-time clinics, located in New York City’s South Bronx, Corona, Queens and Central Harlem communities. Founded in 1974 by Dr. Richard Izquierdo, a local physician, they partner with our school to ensure our students are fufilling Doc's mission. UHP provides comprehensive and affordable primary and specialty health care services to all the communities we serve and houses a school-based health clinic inside of our school itself!
